YOUR WORKERS' COMPENSATION CLAIMS GUIDE

To get started, download our worker's comp claim kit.

  1. Download. Review the Claims Kit Packet and complete the WC-1. Please be sure to carefully read the instructions and complete the appropriate form(s) based on your situation.
     
  2. Report Your Claim:  
    1. Call us at 808-527-7711 or 800-272-5202 toll free with the information on the WC-1 or,
    2. Fax the WC-1 to us at 808-543-3120 or,
    3. Email the WC-1 to us at claimsreportsdesk@ficoh.com

  3. Claim Review
    1. A dedicated Workers' Compensation adjuster will contact you to investigate the accident/injury and to determine compensability under your workers' compensation policy.
      1. If warranted, contact will also be made with your injured worker and any medical treatment facilities they may have visited due to the injury
         
  4. Claim Decision
    1. Payments - Claim payments on file include but are not limited to:
      1. Medical care subject to the state's workers' compensation Hawaii Fee Schedule
      2. Lost time to your injured employee if they are unable to work due to the injury 
    2. Working towards a conclusion
      1. The adjuster will ensure your injured worker receive the treatment to resolve their injury and if applicable, get them back to work
      2. Once the injury has resolved, the claim will be closed.
